Just call our payroll department at 512-453-8833 and ask to have the form sent to you with your next paycheck.

Below you can download a copy of the Peak Performers timesheet for printing purposes. This is not an electronic timesheet (stay tuned for that) so you will need to print, fill out, sign and have it signed by your worksite supervisor, then fax it back to us. Be sure to make a copy for both you and your worksite supervisor. Then, as always, be sure to call our payroll department to verify that we have received and can read it.
